Starting in January 2013 EDRS will begin assigning a State File Number (SFN) to an Ohio death record once it is saved as “Complete” in EDRS. NEW! Attention Coroner and Funeral Home Staff: EDRS has been upgraded to automatically assign a unique State File Number (SFN) to death certificates saved in a "complete" status in 2013. This will affect all death certificates, affidavits, and medical supplements for deaths which occurred in 2013. The SFN will appear in the upper right corner of the death certificate after it is saved in "complete" status; this unique identifier is the ultimate tracking number within the state archives. Every death record which is assigned a SFN must either be filed or voided. If an error on a completed death certificate cannot be corrected with an affidavit or medical supplement, a request can be made to the VS HelpDesk at 614-466-2531, option 3. This upgrade in EDRS does not change the process for completing a death certificate. The electronic verification of "decedent name + date of birth + SSN" will continue to occur after a record is saved in any status. This upgrade does not change the process for printing, signing and filing a complete death certificate with the local health department. |
