New VA Requirement. When ordering a headstone, marker, or medallion for a veteran’s grave in a private cemetery, a funeral home needs to file VA Form 40-1330 or 40-1330(M). The VA has now imposed an additional requirement when a funeral home files this form. The funeral home must include with the form a written instrument showing that the family has appointed the funeral home to represent the family and obtain a marker or medallion on their behalf. NFDA has prepared a new form for a funeral home to document its appointment by the family to process a claim for a government headstone, marker, or medallion.
